APPLICATION FOR VOTING MEMBERSHIP

Dear Rising Phoenix friends and members, begin or renew your membership here! Membership is $25 annually and includes voting rights in our annual meeting.

Outlined below is the criterion that must be met in order for membership to be considered. We are confident that you will find being a voting member of Rising Phoenix Church a rewarding and enriching experience.

Criteria:

Must be at least 18 years of age
Must declare his/her acceptance and belief in the science, philosophy and religion of Spiritualism, consistent with the teachings of the Church.
Acknowledge proof of the continuity of life.
Must volunteer one hour per month at a Rising Phoenix Church sponsored service/event.
Must actively attend two Rising Phoenix Church service/event per month.
Must pay membership fee of $25.00 (cash, check, PayPal) per year. Renewal will be by the end of July of each year. Will not be prorated.
Must be recommended by an active member of the Church.
Membership closes 30 days prior to the Annual Meeting.

Benefits of Voting Membership
If approved, you will be eligible to attend and vote on business items at the Annual Church Business Meeting.

Note: Membership may be cancelled, modified changed at any time by the Board of Directors without notice.
